We are looking for a Regional Care Management Director to provide leadership and advisory services at an enterprise level to client hospitals across the Midwest to help improve Care Management departments and enable the care management software the hospitals have purchased to work optimally.  This is a full-time, permanent position with the healthcare software company, offering a competitive salary, excellent benefits and career growth opportunities. 

A Regional Care Management Director provides leadership and advisory services at an enterprise level to deliver care management strategy across the healthcare continuum. In this role, the director will:

- establish care management goals, strategies and objectives that support population health management and the transition to emerging payment models;

- design, implement and evaluate care management programs to ensure a clinically integrated patient-centered model of care across the enterprise;

- develop risk stratification methods that ensure appropriate focus and use of resources to manage a complex patient population across the continuum.

Additional responsibilities include:

- develop care management models and workflow designs and integrate them into practice and collaborate with medical leadership to integrate evidence-based clinical guidelines and protocols into care coordination processes.

- Through strong communication and interpersonal skills, the director will provide leadership on development and implementation of utilization management plans and strategies to improve reimbursement outcomes, ensure appropriate level of care designation and optimize utilization of resources.

- Educate physicians and other clinical team members about CMS regulatory changes will also be a main responsibility of this role.

Keys to the job:

  • Bachelor’s degree in Nursing
  • At least 8 years of clinical or related healthcare work experience
  • At least 8 years of care management or chronic disease management experience
  • At least 5 years of hospital discharge planning OR utilization review experience (must have both Acute and Ambulatory care experience in this field)

A national healthcare IT corporation that operates more than 40 health centers nationwide (in addition to their leading EMR business) has asked us to find them a permanent Nurse Practitioner Manager for their Irving TX Health Center. This center has been set up for a large, local employer and takes a proactive, educative approach where associates and their dependents are motivated to adopt healthy lifestyles and practices. Focused on wellness and providing Urgent Care, Primary Care, and Occupational Health services to corporate employees and their dependents, this health center is unlike any other medical practice or corporate health center.

As Health Center Manager, Nurse Practititioner you will have both clinical and staff management responsibilities. You will conduct exams and develop treatment plans for injuries, acute illnesses, and chronic conditions. This includes performing diagnostic testing, prescribing medications, and performing minor surgical interventions, when needed. Patient education is a big focus, so you will play a key role by counseling patients on preventive care, medical issues, and use of prescribed medications. Your staff management activities include proactively addressing issues, determining schedules and allocating resources appropriately, and assessing performance to ensure effective patient care. You will also provide coaching around performance, complete performance reviews, and provide development planning, while creating a respectful work environment.

Basic Qualifications:

  • Master's Degree in Nursing or equivalent relevant work experience 
  • Licensed as a Nurse Practitioner 
  • DOT certified or some experience in occupational health with a willingness to become DOT certified before starting this position
  • Health Care operations management work experience or nursing shift leader/charge nurse

About the job (in short)…
Health system needs someone who has been a CIO or CEO of a hospital system who would enjoy more freedom to do deep study, problem solving, advising and relationship building system wide versus running direct reports/a department. This person needs to have Epic EMR implementation experience, however this should not be their primary skill, nor what they are primarily doing now. The ideal candidate would have moved up from this responsibility.  This position is more about relationship management than IT implementations/operations.  
 
What you’ll do

As the Vice President Regional CIO, you’ll function as a business relationship manager being the liaison between corporate enterprise IT Service Delivery leadership and the local hospital IT leadership. This is not your typical hospital CIO role as you will not individually establish and carry out the IT strategic vision, and the technical teams do not report up through you. This is an individual contributor role. You will however, work with business leaders across the system to identify needs for changed or new IT services, advise the hospital business leaders on the best ways to meet their need, then manage the request for new or changed service through the ITS governance process. Additionally, you will be accountable for all IT activities delivered for the region. You will partner with the executive teams to localize the IT strategy, harmonize roadmaps with local plans, and optimize delivery to maximize the value of IT and customer satisfaction for the region. In order to be successful, it will be imperative to engage regional stakeholders to identify and align on priorities, opportunities, and commitments; ensure on-going communication across the region on accountabilities, progress, deliveries, and changes; and ensure the National ITS leaders understand the regional priorities and opportunities, and are engaged.

You can make the biggest impact by meeting these requirements:

  • Health care provider experience required within a large health care system, not just an individual hospital.
  • Bachelor’s Degree in related field required. Master’s degree in Business or Information Technology preferred.
  • 10+ years of large enterprise IT leadership experience in a highly matrixed environment.
  • 5+ years’ experience deploying health care enterprise applications, Epic required.
  • Successful experience with large offshore IT service providers.
  • Ability to build IT and business consensus that is supportive of driving change.
